Medical Athletic Forms
Please have all your Medical forms turned in and ready by the first day of Practice (Aug. 2 for Varsity) (Aug. 13 for JV). Your physical must be updated or current within a years date. If you do not have one in that criteria, you will not be able to participate. There are new regulations and rules that require this for all sports and there will be no way around it. By then end of the week, I will have a list based off of last year and will be able to let you know when your date is expiring for your physical. Contact me if you have any questions.
